Untreated patients with CTEPH progress to right heart failure and death, 7 albeit generally slower than patients with PAH.8 Concomitant small vessel arteriopathy is present at various degrees in patients with CTEPH.9 In approximately 10% of operated patients, pulmonary hypertension does not resolve despite removal of significant amounts of organised thrombus. Introduction: A few studies only has recently reported that the incidence of apoptosis in cultured mammalian granulosa cells GC ; was increased by gonadotrophin releasing hormone GnRH ; agonists and decreased by FSH. GnRH agonists and FSH are widely used in controlled ovarian hyperstimulation protocols of human assisted reproduction. The aim of this study was to investigate the effects of the GnRH agonist triptorelin, an agonist commonly used in France, and of the recombinant FSH, on the induction of apoptosis in human luteinized GC isolated from women undergoing IVF. Materials and methods: Follicular fluids FF ; of patients aged 3239 years undergoing IVF were obtained at the time of oocyte retrieval by follicle aspiration and pooled. Indications for infertility treatment were bilateral tubal obstruction and male infertility. Red blood cells were removed from follicular aspirates by centrifugation through gradient density. White blood cells were depleted by treatment with anti-CD45 coated magnetic beads. Luteinized GC were dispersed by incubation with trypsin. Trypsin digestion was stopped after 5 mins by addition of MEM supplemented with 10% fetal calf serum FCS ; and antibiotics. Purified GC were cultured overnight at 37C under 5% CO2 and air in 10% FCS medium. At day 1, plated GC were incubated for 24 h with i ; serumfree medium, ii ; serum-free medium supplemented with FSH 100 ng ml; 500 ng ml ; , and iii ; serum-free medium supplemented with triptorelin 100 ng ml; 500 ng ml ; . day 2, trypsin-generated GC suspensions including floating cells were fixed in 70% ethanol, treated with RNase A, stained with propidium iodide and analysed by flow cytometry. GC with less than 2 n DNA content in the cell cycle analysis profile were considered to be apoptotic and counted. Results: Defining the incidence of apoptotic GC after 24 hour incubation without FCS and any hormone addition as 100%, the percentages of subdiploid GC after hormone treatment were respectively: i ; FSH 100 ng ml, 29%; FSH 500 ng ml, 29%, corresponding to a 71% diminution of apoptosis, and ii ; triptorelin 100 ng ml, 184%; triptorelin 500 ng ml, 865%, resulting in a 1.8- and 8.65-fold increase of apoptosis in human GC. Conclusion: This present study demonstrated that FSH decreased the apoptotic phenomena in human luteinized GC and apparently without dose dependance, at least in the limit of our protocol. In contrast, triptorelin was implicated in a dose-dependent manner increase of the incidence of apoptotic GC.
